This form is used to request undercover license plates for documented, specific law enforcement investigations and surveillance purposes or
for documented, specific security purposes. As provided under Section 40-12-250, Code of Alabama, the license plate issuing official must
ensure that the requesting law enforcement agency has obtained the appropriate state, county or municipal license plate from the Department
of Revenue prior to the issuance of an undercover license plate. Per Executive Order No. 728 (Jan. 17, 2023), page 1 of this form must be
signed by the department or agency head, and each department or agency must retain a complete copy of the form in its files as
confidential law enforcement or security records.

For each vehicle listed on page 1 of this form, the department or agency must provide written documentation for the necessity of an undercov-
er tag. Each justification must state a documented law enforcement surveillance or undercover investigative need or a documented security
need. Example: “Will be used in undercover drug trafficking interdiction operations” or “User of vehicle has received security threats based on
position.” This page must be retained in the agencyʼs file with the completed form.
